Passport correction – fix passport errors, Data or Name Change

Changing information on passport is a very common occurrence. Some reasons are printing error by passport agency or a person legally changes there name for example a marriage.

If you look at your passport and the date of birth is wrong or you have wrong sex on there or any info needs changing you must do a passport correction. To do this you use the DS-5504 application. This enables you to change the descriptive data on your passport.

To qualify for name change or data correction service one of the following is required.

  • You need to change your name in your valid U.S. passport
  • You need to extend the validity of a limited passport (if eligible)
  • You need to correct a printing error in your valid U.S. passport
